Full Job Description Description:
The social services designee is someone that represents the interests of the residents and the families of St. Jude’s. This is the primary person responsible for the psycho-social well-being of the residents at St Jude’s. This person is an unbiased lesion with understanding of the residents’ rights. This person is also the representative to filter grievances and requests not only from residents and families but also from other facilities and organizations in our community. This person should ensure that all residents’ needs and “achievable” wants are identified and addressed on an ongoing basis. This person works with all departments to maintain a happy resident population.
Daily:
- Monitor through interviews and floor rounds all “dignity Issues” for residents. nail care, hair care, clothing repair, state of rooms, staff resident interaction etc.
- MDS Assessments

Notes:
MDS info, behavior, room changes and ongoing resident or family complaints that dictate documentation
- Manage resident requests: Popular topics include cable issues, phone issues, store runs for special supplies, IOP, change of PCP, roommate issues, lost/ stolen items.
- Social History & admissions note for chart
- Investigate Incident Accidents and record into a spread sheet Weekly
- Manage any family or resident complaint and pass on the issue to the proper department if you are unable to find resolution i.
e., Nursing Issues go to Kathe, complex grievances need to go to Alec. Work to anticipate where an issue may arise and attempt to alleviate this through coordinating family and administration meetings, administrator and resident meetings, hospice family meetings, pastoral care visits. Importantly, assist with implementation of solutions.
- Grievance Log upkeep: record all reported grievances spot check with residents to ensure resolution
- Attend care plans and address any resident requests that do not fall into the category of dietary, therapy and nursing.
- Bed holds letters
- Monitor troubled residents with transition from home to NH; assist with this adjustment
Monthly/ As Needed:
- Scheduled discharges
- My place Louisiana for residents who need assistance with waiver programs
- Help coordinate meetings for families that may wish to consider Hospice or need more info about this
- Help residents that discharge to psych. by reviewing social worker recommendations and implementing whatever suggestions that will help resident, i.e., put balloons on room door for residents that wander into to other people’s rooms.
- Know all resident’s payer sources and assist with enrolling and dis-enrolling where necessary.
- Apply any resident without social security for it.
- Respond to all “nonclinical” Dr. Orders as well as make referrals to outside agencies where necessary, IOP, fixing of electric chairs, help with transport auths. (Partner with Ruth and Alec on these type of situations)
- Communicate with all families that have loved ones that have passed or been discharged and need to come picks up personal items from their room: Allow 48-hour time frame then work with Maintenance to move the items.

Family Support Group Quarterly:
- Family Council Meetings
- Assist with identifying residents that need eye, vision, dental or podiatrist attention
Yearly:
- Manage Hurricane preferences; these should be updated or confirmed with families before each Hurricane season.
- Manage list of “critical need” residents received from nursing staff during hurricane season in order to keep track of the number of residents traveling via ambulances and staying in nursing homes.
- Assist residents with voting, process & registration.
