We are expanding our team and have PRN (can lead to FT) opportunities on the Northshore. Overview We are seeking a compassionate and dedicated Registered Nurse for our Home Health division. The ideal candidate will possess strong direct patient care skills. This role requires a commitment to providing high-quality care to patients, ensuring their comfort and well-being while promoting independence. Duties Provide comprehensive nursing care to patients in their homes, including assessments, planning, implementation, and evaluation of care plans. Administer IV infusions and medications as prescribed by physicians. Collaborate with multidisciplinary teams to develop individualized care plans tailored to each patient’s needs. Conduct triage assessments to determine the urgency of patient needs and coordinate appropriate responses. Monitor patient progress and adjust care plans as necessary based on clinical findings. Educate patients and their families on health management, medication administration, and lifestyle modifications. Maintain accurate and timely documentation of patient care activities in compliance with regulatory standards. Qualifications Current Registered Nurse (RN) license in the state of Louisiana. Minimum of 1 year of nursing experience. Strong clinical skills. Excellent interpersonal skills. Ability to work independently and manage time effectively. Strong communication skills, both verbal and written, with the ability to educate patients effectively. Excellent organizational skills with attention to detail in managing multiple patient cases.
