Your Mission:

Orchestrate Seamless Recovery As a Case Manager at Oasis Point, you are the “captain of the ship” the vital leader who ensures all patients care plan is expertly coordinated, clinically sound, and compassionately delivered. You will oversee the entire continuum of care, collaborating with an elite interdisciplinary team, advocating for your patients, and driving both quality and financial performance. Key Responsibilities Comprehensive Care Coordination Conduct and document thorough initial assessments within 72 hours of admission, including psychosocial, clinical, and family disposition status. Develop, implement, and continually update individualized care plans in partnership with patients, families, and the interdisciplinary team. Lead regular team conferences, synthesizing input from therapy, nursing, and medical staff to ensure unified, goal driven care. Length of Stay & Utilization Management Proactively manage length of stay, ensuring compliance with Medicare (CMS) guidelines and the 60 percent rule. Oversee insurance reauthorizations, facilitate the QIO process, and ensure timely, appropriate service delivery. Serve as the primary liaison to payors, external case managers, and regulatory bodies. Discharge Planning and Family Engagement Coordinate all discharge activities, including equipment needs, patient family education, and post discharge services. Ensure smooth transitions of care, with proactive follow up to assess outcomes and additional needs. Quality, Compliance, & Leadership Participate in quality of care reviews, policy development, and performance improvement initiatives. Educate team members on cost containment, regulatory updates, and evidence based practices. Champion ethical, legal, and patient centered standards in every aspect of care delivery. Qualifications Licensure and Education Current Louisiana licensure as an RN or LPN required. Bachelors degree or higher preferred; equivalent experience considered. BCLS certification required. Experience Minimum two years of case management experience in an IRF or acute rehabilitation setting. Demonstrated expertise in length of stay management, discharge planning, and interdisciplinary collaboration. Skills and Competencies Exceptional communication verbal and written in English. Proficiency with EMRs and electronic billing procedures. Strong organizational, leadership, and problem solving abilities. Deep understanding of CMS IRF regulations and utilization review processes. Physical Demands and Work Environment Regularly required to sit, stand, walk, and communicate effectively. Occasional lifting of up to 20 pounds.
