Registered Nurse Part Time Job Summary:

Responsibilities:

• Collaborate with physicians and healthcare providers to develop and implement patient care plans
• Review and assess patients’ medical records to evaluate their condition and progress
• Provide personalized support and comfort based on individual patient needs
• Assist in discharge planning and coordination of care transitions
• Work with patients’ families to ensure comprehensive care delivery
• Manage medication reconciliation and accuracy of medication profiles
• Notify the appropriate medical personnel of any changes in patient condition
• Maintain accurate patient records in the electronic medical records system
• Provide emotional support to patients and families throughout the care process
• Perform quality assurance and chart audits to ensure compliance with regulations
• Any other duties as assigned by the healthcare administration

Requirements:

• Active nursing license
• Experience in care management, critical care, outpatient, inpatient, OR, and discharge planning preferred
• Strong communication skills and compassionate nature
• Ability to work autonomously and demonstrate leadership skills
• Knowledge of nursing care processes and medication guidelines
• Familiarity with EMR systems such as Kinnser/WellSky is a plus

Qualifications:

• Minimum of two years nursing experience
• Previous experience in hospice, home health, or oncology settings is advantageous
• CPR certification required
• Legally authorized to work in the U.S.
• Strong observational, verbal, and written communication skills
