Job Description
Nature of Duties:
The goal of the Structural Heart RN is to promote seamless care from referral to discharge. The Structural Heart RN works in direct collaboration with the cardiologists, cardiovascular surgeons, Structural Heart Coordinator, nurse practitioners, and other team members; and participates in the ongoing assessment of valve patients pre and post-op. The RN provides and promotes patient education and serves as a liaison between physicians, patients, families, nursing staff, and referring facilities to promote quality and continuity of patient care.
Qualifications:
Graduate of an accredited school of Nursing Current licensure from the Louisiana State Board of Registered Nurses Basic Life Support Certification Advanced Cardiac Life Support Certification A minimum of 2 years of nursing, preferably in cardiology
Specific Duties :
Assists in the development, implementation, and revision of nursing protocols and standards of care. Develops patient education programs and tools. Researches and provides advice on patient care processes, nursing processes, and larger-scale quality improvement projects Assesses the patient’s condition and needs using critical thinking and clinic judgment implements appropriate nursing actions and collaborates with other disciplines to meet the patient’s/family’s physical, emotional, and other needs; and evaluates patient’s progress Serves as an assistant to the Structural Heart Coordinator for Structural Heart patient visits and records patient’s health information and establishes records of care. Participates in Pre-, Peri- and Post-Procedure activities directly involving interactions with the patient and/or family; can include but is not limited to education, testing coordination, medical record review, status reports, follow-up phone calls, and other activities determined to be relevant: a. Act as a point of contact for patients, families and referrals, including telephone triage. b. Assist with the scheduling for Structural Heart Cases to create efficiency and improve effectiveness for the cardiologist. c. Follow patients through the continuum, including Pre-admission testing, inpatient admission, 30 day and one year follow up, or as required by a research protocol d. Reinforce patient education and directs patients and families to available resources and support services. e. Ensure patients receive a treatment plan that is understandable and feasible. Assess for clinical, emotional, spiritual, psychosocial and other patient/family needs. f. Assures all Medicare requirements in National Coverage Decision for TAVR and other structural heart procedures are completed. Coordinate timely scheduling of tests, procedures, and appointments. 6. Consults and collaborates with members of the health care team in the assessment and management of health care problems; functions as a liaison between physicians, nursing, technologists and area support staff. 7. Assists the Structural Heart Coordinator with developing patient education programs and tools. 8. Researches and provides advice on patient care processes and larger-scale quality improvement projects. 9. Develop case review presentation for case review meetings. 10. Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.): a. Building sustained relationships with referring physicians. b. Following up with referral following cases c. Attends conferences and appropriate investigator meetings and networking with other centers. 11. Responsible for TAVR or any other structural heart registries: a. Work closely with data analyst to collect TVT registry data, track outcomes and support strategic planning processes. 12. Adheres to and supports team members in exhibiting CIS values of integrity, community, compassion, and dedication. 13. Adheres to CIS organizational and department-specific safety and confidentiality policies and standards. 14. Performs related duties as assigned.
