• Case Management Nurses ( RN) coordinate long-termindividual care services beyond the bedside, providing long-termassistance to patients and family members to advocate for theirquality care. Case Mgmt RN job responsibilities include, but arenot limited to:

• Requirements:

• Pair patients with resources theyneed within or outside current facilities, finding affordable,efficient healthcare services that suit every patients uniquemedical condition
• Document case management plans while settinggoals for each patients treatment
• Case Management RN Collaboratewith a variety of healthcare providers offering the resources tohelp their patients
• Requires 2+ years of recent Case Management RNexperience
• Experience in Discharge Planning in Acute CareHospital
