Duration:

13 Weeks contract

Location:

Hyannis, MA Shift

• 11:00 AM
• 9:30 PM or 11:30 AM
• 10:00 PM | 40 hours/week | Four (4) 10.

5-hour shifts

$2,900 weekly include stipend

REQUIREMENT

BLS Job Duties:

Conduct daily screening of new admissions to assess discharge planning needs. Verify demographic and payer information; notify admissions of necessary corrections. Review and evaluate admission appropriateness and level of care (Inpatient vs. Observation). Identify and refer high-risk patients with complex psychosocial, financial, or legal needs. Facilitate communication with third-party payers to support concurrent clinical information requests. Participate in and facilitate daily rounds; collaborate with physicians and care team on care and discharge planning. Act as a patient advocate, providing information regarding insurance coverage and discharge planning. Coordinate referrals and ensure complete and accurate transfer of information to external facilities or services. Apply knowledge of community resources and act as a resource to care teams. Evaluate and monitor patient progress through clinical pathways and Care Maps. Work with medical coders to ensure accurate diagnosis and procedure coding. Maintain compliance with regulatory requirements, infection control, and safety standards. Stay updated on current trends in case management and payer regulations through ongoing education. Display initiative, independence, and strong time management in daily responsibilities.

Required Qualifications :

Current RN or LPN license in the Commonwealth of Massachusetts Minimum 3 years of acute care experience in the past 5 years in a hospital or case management setting BLS Certification (required) Strong organizational, communication, and negotiation skills Ability to manage multiple priorities in a fast-paced and high-stress environment Proficient in computer systems and clinical documentation software
