Job Description

Under the direction of the clinician, department supervisor or designee,provide the highest standards of clinical support to the MACC practice.

Delivers excellent customer service to patients through participation ina medical care team. Demonstrates a high level of competence withclinical support in accordance with MACC?s policies & procedures.

Description Monitor the Electronic Health Record nurse pool throughoutthe day & respond in a timely manner Triage incoming calls, messages, &In-Basket tasks, being able to prioritize urgent matters & scheduleappointments as needed Exercises thinking, nursing judgment, & standardtriage guidelines to ensure health needs & concerns of a patient, group,or population are met Assesses complex patient needs to ensure patientis transitioned to appropriate level of care Demonstrates a systematic,dynamic approach to collect objective & subjective data & assesses theresponse Triages & schedules visits based on assessment of patientdisposition needs & consideration of available & appropriate resources;refer for emergency & follow up care Provides clinical support to team,adjusting role & priorities to meet patient care demands Answers phonecalls & electronic messages promptly Greets callers appropriately byidentifying self & department, & provides assistance in a friendly,respectful, & helpful manner Provides timely, efficient, & accuratetriage responses in accordance with professional nursing standardsDocuments all calls, encounters, & actions taken into the patient?smedical record timely & completely according to established policy ≺ofessional standards Educate patients about utilization of theclinics? community services Collaborate with the interdisciplinary careteam as needed to avoid interruption of delivery of services to patientsPerforms other work-related duties & activities as assigned or requestedGreets & escorts patients to exam rooms Obtains relevant information &takes vital signs as indicated (i.e. height, weight, blood pressure,heart rate, respiratory rate, oxygen saturation). Inputs pertinentclinical information into the electronic health record timely & withaccuracy & performs a complete medication reconciliation Assistsclinicians with clinical procedures &/or additional tests specific tothe department. May advise patient of preparation required for exam oradditional testing & keeps patients informed of visit status, unforeseendelays & other relevant information pertaining to the patient?s visitAssists in resolving patient concerns by acting as an advocate, asappropriate Participates in practice & department meetings, trainings,etc. to promote continuous improvement & quality patient care Takesinitiative to learn, understand, & stay current with community resources& the greater healthcare system while continuously participating inself-assessment of skills Maintains patient rights & abides by allconfidentiality obligations pursuant to

CCHC & HIPAA

Providesclerical/office support as necessary (i.e. scheduling appointments,greeting & registering patients, scanning incoming medical records &paperwork, preparing medical records, answering phone calls & takingmessages, faxing, etc.) Ability to float to other MACC practices whenrequired Qualifications RN (X21A) –

EDUCATION/EXPERIENCE/TRAINING:

Ability to read, write & communicate in English Graduate of anaccredited school of nursing Currently licensed as a Registered Nurse inthe Commonwealth of Massachusetts Basic Life Support Certification forHealth Care Providers Experience with current office technology ≺oficient with computer office applications (Electronic Medical/HealthRecords, MS Office, etc.) Minimum of two years’ experience in aclinical setting preferred
