Job Summary:

/n A Wound, Ostomy, and Continence (WOC)Registered Nurse (RN) provides specialized care to patients withcomplex wounds, ostomies, and continence-related issues. WOC RNsassess, plan, and implement individualized care plans to promotewound healing, manage ostomy care, and support patients withcontinence challenges. They collaborate with multidisciplinaryteams to ensure comprehensive care, education, and support forpatients across various healthcare settings. /n/n

KeyResponsibilities:

/n /n

• Assess, diagnose, and manage patients withchronic or acute wounds, including pressure ulcers, diabetic footulcers, surgical wounds, and traumatic injuries. /n
• Develop andimplement individualized wound care treatment plans, including theselection of appropriate wound dressings, therapies, andinterventions. /n
• Provide ostomy care, including pre-operativeand post-operative education, stoma care, and management ofcomplications such as leakage or skin irritation. /n
• Educatepatients and their families on wound care, ostomy management, andstrategies for promoting skin health and preventing complications./n
• Collaborate with healthcare providers, dietitians, andtherapists to develop comprehensive care plans for patients withwound, ostomy, and continence needs. /n
• Monitor and assesspatients’ skin integrity, identifying and managing pressure ulcers,skin breakdown, and other wound-related conditions. /n
• Providecounseling and emotional support to patients and families,addressing concerns related to wound care, ostomy care, and qualityof life. /n
• Maintain accurate documentation of patientassessments, care plans, interventions, and outcomes. /n
• Advocatefor patients in accessing wound care supplies, medications, andappropriate wound or ostomy products. /n
• Participate in wound andostomy care education and training programs, as well as staffeducation sessions to promote best practices. /n
• Assist with datacollection, quality improvement initiatives, and the development ofevidence-based care practices related to wound, ostomy, andcontinence care.

Qualifications:

/n /n

• Education:

  Bachelorof Science in Nursing (BSN) preferred; Associate Degree in Nursing(ADN) accepted. /n

• Licensure:

  /n /n

• Valid Registered Nurse (RN)license in the state of practice. /n /n /n

• Certification:

  /n /n

• Wound, Ostomy, and Continence Nurse (WOCN) certification preferred./n
• Basic Life Support (BLS) certification required. /n /n /n
