Summary:

This is Monday through Friday 11a-7p position does include someon-call responsibilities. The primary purpose of this position is toprovide preadmission clinical review of patient referrals and tocoordinate the discharge process.   • Essential Job Functions:

  • Maintains resident census consistentwith the admission strategy with Admission Coordinator
  • Develops goodrapport and working relationships with IHCS Liaison; area dischargeplanners; and appropriate social agencies to promote the facility’sreputation and encourage referrals.
  • Facilitate and/or direct admissionprocess
  • Be an advocate and spokesperson for resident concerns
  • Complete nursing assessment, care plan, and document.
  • Provide reportto nursing staff.
  • Gather, document and obtain MD order for admissionmedication list.
  • Complete medication reconciliation.
  • Ordermedications from pharmacy.
  • Responsible for initiating Medicare formson admission.
  • Responsible for consents.
  • Identify potentialroom-review for any infection i.

  • Identifyequipment needs
  • Identify clinical issues
  • Notify Admissions ofdecision
  • Greet resident to appropriate room
  • Do all assessments foradmission process and write nurses note
  • Develop care plans based onassessments, physician orders
  • Create C.N.A Care Card
  • Report onresident to Nurses and C.N.A’s
  • Complete/facilitate discharge planningand discharge process.
  • Write referral to transferring agencies
  • Assure patient has a 7 day supply of medications
  • Observe dischargeorder obtained and written
  • Review discharge paper work in place anddischarge note
  • Qualifications
  • Must possess, a Massachusetts RNLicense
  • Minimum of 2 years clinical nursing experience.
  • Must be ableto read, write, speak, and understand the English language.
  • Mustpossess the ability to make independent decisions when circumstanceswarrant such action.
