Responsibilities:

Under the direction and supervision of the Director of Supportive Care Services or designee, perform a variety of duties both within and in support of Hospice and Palliative services including, but not limited to; closely collaborating with both inpatient and outpatient staff to transition patients to Hospice and Palliative Services, discussing goals of care with every patient and documenting such conversations, educating the physicians, referral sources and community about the services available within the Hospice and Palliative Care Team, acting as a liaison with referral sources and identifying the benefits of hospice services to the community and potential clients. The Conversation Nurse is also responsible for census development within the hospice program, including increasing hospice census, increasing Length of Stay and tracking referral source needs and feedback.

Qualifications:

Graduate of an accredited School of Nursing is required and a Bachelors Degree in Nursing is preferred. Current RN licensure in the Commonwealth of MA and RI is required. Certification in Hospice and Palliative care preferred. Some experience in hospice or home care field required and a minimum of two (2) years experience is preferred. Demonstrated self motivation to plan, organize and achieve own work objectives is required. Must possess and demonstrate highly effective communication and customer service skills. Must be able to blend strong clinical skill with excellent problem solving and organizational skills. Requires proficient computer skills, to include electronic tracking of referrals, report development, including experience with various software, database and electronic records programs. Current RN Licensure in Rhode Island and the Commonwealth of Massachusetts is required. Hospice and Palliative Certification Preferred. Basic Life Support (BLS) certification is also required. Must be fully vaccinated against seasonal Influenza and the

virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired

Compensation:

Pay rate will be determined based on level of experience.

Pay Range:

USD $86,444.80 – USD $153,732.80 /Yr.
