Great Life work Balance position with excellent benefits The RNCase Manager provides comprehensive case management for assignedSCO members with complex needs to ensure cost effective quality ofcare. This professional provides quality patient care via thedevelopment, implementation & evaluation of individual patient careplans. This individual will adhere to established professionalstandards of care, and the Element Care policies and procedures.

This is a full-time position, Monday

  • Friday 8-4 position; nonights, weekends or holidays!
Responsibilities:
  • Develops a planof care that corresponds with the physicians’ orders throughassessment of patient’s needs, condition, environment, andconsultation with other health team members, at time of initialvisit and updates plan of care regularly as necessary and documentsthis care following Element Care procedures.
  • Ensures the deliveryof competent, quality patient care in the home.
  • Maintains anongoing responsibility for assigned caseload.
  • Oversees theimplementation and evaluation of individual patient plans of care
  • Maintains communication with Commonwealth Care Alliance (CCA) andElement Care staff as well as the provider community as appropriate
  • Provides patient education opportunities.
  • Works with bothmembers and their family members
  • Actively participates in teamknowledge sharing activities.
  • Performs other duties, as assigned.
Qualifications:
  • Registered Nurse currently licensed in theCommonwealth of MA
  • Bachelor of Science degree in Nursingpreferred.
  • Minimum of three (3) years experience as a licensed,registered nurse, preferable five (5) years, including home careclinical experience.
  • Proficient in the use of Microsoft Officetools.
  • Experience working with case management data systems.
  • Ability to travel locally on a frequent basis.
  • Covid vaccinerequired.
  • Spanish speaking preferred
