Role Summary:

Join a dynamic care coordination team as a Travel RN Case Manager. This role focuses on assessing patient needs, optimizing utilization of services, and ensuring smooth care transitions for hospitalized patients.

Primary Responsibilities:

Screen and assess patients on admission for discharge and care planning Use InterQual® to conduct utilization reviews within defined timelines Collaborate with medical teams to support timely and appropriate discharges Address payer requirements and communicate appeal status Identify patients with high-risk needs and connect them to appropriate resources Ensure timely response to insurance documentation needs Guide discharge planning and coordination with external providers Track patient progress during interdisciplinary team discussions Monitor care quality and contribute to continuous improvement efforts Educate patients and families on post-discharge care and follow-up Stay current on regulations, quality standards, and hospital policies

Required Qualifications:

MA RN license in good standing Bachelor’s degree in nursing preferred Minimum 3 years in hospital or case management nursing InterQual® or comparable experience essential Knowledge of discharge protocols and payer guidelines Strong documentation and EHR proficiency Certified Case Manager (CCM) or related credential a plus
