  • The RN:
  • leads the healthcare team in providing quality patient care.
  • assesses, plans, implements and evaluates patient care from admission to discharge.
  • directs, assigns, and supervises ancillary staff team members in providing the appropriate level of care in the most efficient manner.
Professional RN Responsibilities:

Provides patient-centered care1)

Assessment:

The RN collects health data.

Performs complete and thorough assessments of the patient’s physical, functional, spiritual, cultural, psychosocial, nutritional, and pain status.

Involves the patient and significant others in the assessment process.

Conducts reassessments according to current nursing standards.

Documents assessments according to current nursing standards.2)

Diagnosis:

RN analyzes the assessment data in determining diagnoses.

Derives nursing diagnoses or problems from the assessment dataValidates diagnoses with the patient/significant others as possibleDocuments diagnoses to facilitate development of expected outcomes and the plan of care.3) Outcome identification: The RN identifies expected outcomes individualized to the client.

Develops outcomes that are measurable and realistic.

Involves patient/significant other in the formulation of outcomes, when possible.

Uses outcomes to direct the plan of care.4)

Planning:

The RN develops a plan of care that prescribes interventions to attain expected outcomes.

Individualizes the plan of care to the patient’s needs and condition.

Develops the plan with the patient/significant others, when possible.

Utilizes current nursing knowledge and evidence to plan care.

Consults and collaborates with other disciplines to meet identified patient/family needs.5)

Implementation:

the RN implements the interventions identified in the plan of care.

Provides interventions that are consistent with the plan of care.

Implements interventions in a safe and appropriate manner.

Documents interventions according to current nursing standards.6)

Evaluation:

the RN evaluates the patient’s progress toward attainment of outcomes.

Systematically evaluates the patient’s response to interventions.

Evaluates the effectiveness of interventions in relation to outcomes.

Uses ongoing assessment data to revise diagnoses, outcomes, and the plan of care as appropriate.

Communicates with other disciplines in evaluating the effectiveness of the plan of care.

Education:

Graduate of an accredited school of nursing.

Experience:

Previous med/surg experience preferred.

License/Certification(s):1. Current Massachusetts RN licensure2.

BCLS/ACLS

(within 1 year of hire)3. Chemotherapy Certification (within one year of hire)
