We are seeking a compassionate and detail-oriented Initial Assessment Nurse / Admission Nurse to conduct home visits and assess individuals for admission into our Adult Foster Care (AFC) program . The ideal candidate will have a strong clinical background and experience in home health, geriatric care, or community nursing.

Key Responsibilities:

Conduct initial assessments of prospective AFC members in their homes. Evaluate medical and functional to determine eligibility. Develop care plans and collaborate with caregivers and healthcare teams. Educate families and members on program benefits, policies, and expectations. Ensure compliance with state regulations and AFC program guidelines . Maintain accurate and timely documentation of assessments and recommendations.

Qualifications:

Active RN license in MA. Minimum 1 Year of expereince in Initial Assessments . Experience in home health, case management, or geriatric care preferred. Strong assessment, communication, and documentation skills . Ability to travel locally for home visits. Knowledge of Medicaid, AFC regulations, and community resources is a plus.
