The Clinical Case Manager will work with aninterdisciplinary team ensuring continuity of care, managing andmodifying treatment plans, discharge planning, reviewing andanalyzing case management reports, etc.

• Must be a RegisteredNurse with active MA RN license
• Prefer 2 years related clinicalexperience
• Certified Case Manager (CCM) preferred
