RESPONSIBILITIES & QUALIFICATIONS
Responsibilities:
- Identifies on-going needs and services of residents in promotion ofthe highest quality resident services to be delivered.
- Ensuresproper follow through, assessment and documentation for residentswith a change in health care services and status.
- AssistsResident Care Director (RCD) in conducting resident assessments attime of move in, 30 days post move in, semi-annually and with anychange in status.
- Conducts monthly wellness visits for allresidents.
- Assesses, monitors, and follows up with residents whohave experienced any fall, accident, or incident; documents andreports as required and directed.
- Documents all pertinentinformation in the resident wellness file.
- Maintainscommunications with resident’s family and or responsible party toensure medical needs for the resident are being met.
- Contactsresident’s attending physician when necessary and/or upon familyrequest.
- Ensures weights and vital signs are obtained monthly foreach resident.
- Assists in maintaining wellness files according toSunrise policies and federal and state regulations.
- Maintainsmedical supplies and emergency kits for the community.
- Providesclinical support and assistance to community team members asneeded.
- Understands and follows infection control practices.
- Partners with the RCD and community team to promote infectioncontrol standards including any resident testing, training, andskills checks.
- Understands and practices the proper method ofattending to and disposing of and the possibility of exposure toblood borne pathogens, bodily fluids, infectious waste, sharpsticks, and hazardous materials in compliance with universal careprecautions.
- Regularly monitors each resident’s medication andtreatment profile to ensure each medication and treatment isadministered as ordered and documented accurately.

- Actively participates in Sunrise QualityAssurance Meetings & Clinical Meetings
- Demonstrates and isknowledgeable in the following key regulations:
- All Federal,State and Local resident care and services regulations

- Resident Assessment/Evaluation process in accordance tostate / province regulatory requirements
Qualifications:
- Graduateof approved college / school of nursing.
- Maintains a currentstate license as a professional Registered Nurse (RN).
- Minimum ofone (1) year experience in home health, assisted living, hospital,or long-term care environment.
- Demonstrates knowledge of nursingpractices, techniques and methods applied to health and wellnessresident care, and federal, state, and local regulations.
- Demonstrates knowledge of good assessment skills.
- Ability tohandle multiple priorities.
- Possesses written and verbal skillsfor effective communication and the ability to teach and guide teammembers to provide quality clinical care.
- Competent inorganizational, time management skills.
- Demonstrates goodjudgment, problem solving and decision-making skills.
- Demonstrates proficiency in computer skills, Microsoft Office(Windows, Outlook, Excel) and Sunrise applications with the abilityto learn new applications.
Ability to work weekends, evenings, andflexible hours, available to our customers at peak service deliverydays and times
