This role supports the integrity of the Adult Foster Care (AFC) program by ensuring that documentation aligns with regulatory requirements, internal policy standards, and industry best practices. The contractor also delivers and documents training programs and recommends improvements to optimize performance. Core Responsibilities 1. Quality Assurance Auditing Audit client charts monthly, including: Admission and intake documentation Initial and annual assessments Care plans and service notes PCP forms and annual physicals Monthly caregiver logs physical and emailed. Maintain and update department-specific documentation checklists to be shared with department leaders and the Program Director.
Conduct monthly QA reports outlining:
Files reviewed Compliance issues found Actions taken or required Recommendations for improved documentation practices 2. Oversight of External Vendor Documentation (e.g., OmniStar) Review external nursing submissions (e.g., initial assessments, reassessments, clinical notes) for accuracy, regulatory compliance, and completeness before payment authorization . Submit findings and payment recommendations to the Program Director and Finance within 3 business days of receipt. Maintain a tracking log of vendor documentation issues and resolution status. 3. Staff & Caregiver Training Coordination Work in collaboration with Human Resources to: Schedule and deliver monthly caregiver trainings Provide initial and periodic training for CMs, CSSs, and Nurses Document all training completions and maintain certification logs (initial, annual, semiannual) Ensure all staff and caregiver trainings are aligned with current MassHealth AFC regulations, internal protocols, and any updates in clinical guidelines. 4. Compliance Monitoring & Best Practice Development Provide ongoing compliance feedback to department leaders and Program Director regarding documentation quality. Make evidence-based recommendations for documentation improvements, workflow updates, and training content based on audit findings. Participate in quarterly policy and SOP reviews to keep internal standards aligned with regulation changes. QA nurse must serve as a backup in the event that field staff nurses are unavailable.
Deliverables & Reporting Schedule Weekly Submission:
Review log of vendor-submitted assessments with payment status recommendations.
Monthly Report:
Compliance dashboard including: Number of files reviewed (internal and external) % of charts fully compliant Common errors/trends Trainings completed and upcoming
Quarterly Recommendations Memo:
Summary of observed compliance trends with proposed improvements in processes, training, or documentation practices. Qualifications Registered Nurse (RN) licensed in Massachusetts (required) 2+ years of experience in quality assurance, documentation review, or regulatory compliance in healthcare or home care settings Prior experience conducting or coordinating clinical training (preferred) Familiarity with MassHealth 130 CMR 408.000 (AFC Program Regulations) Proficiency in EMR platforms, Microsoft Office Suite, and remote communication tools Core Competencies Expert attention to detail in documentation Strong communication and training delivery skills Clinical judgment and regulatory interpretation Timeliness and accountability in reviews Integrity, independence, and reliability
