Initial Assessment / Admission Nurse – Adult Foster Care (AFC)

Pay Range:

$90-100 per

Assessment Position Overview:

We are looking for a compassionate, detail-oriented Registered Nurse to join our Adult Foster Care (AFC) team as an Initial Assessment / Admission Nurse. In this role, you will be responsible for conducting in-home assessments to evaluate the eligibility of individuals seeking admission to our AFC program. The ideal candidate will have a strong clinical background with experience in home health, geriatric nursing, or community-based care.

Primary Responsibilities:

Perform comprehensive in-home assessments for prospective AFC members. Assess medical conditions and functional abilities to determine program eligibility. Create individualized care plans in collaboration with caregivers and interdisciplinary teams. Educate members and their families about AFC program benefits, guidelines, and responsibilities. Ensure all assessments and services are in compliance with state regulations and program standards. Accurately document findings, recommendations, and care plans in a timely manner.

Required Qualifications:

Active Registered Nurse (RN) license in the state of Massachusetts. At least 1 year of experience conducting initial assessments. Preferred background in home health care, geriatric nursing, or case management. Excellent assessment, documentation, and communication skills. Ability to travel locally to perform home visits. Familiarity with Medicaid, AFC program regulations, and local community resources is a plus.
