Full-Time Hospice RN Case Manager Locations:

West Roxbury, MA 02132 Safro Staffing is proud to partner with a compassionate hospice care provider known for maintaining a low patient-to-nurse ratio. We are seeking a dedicated and experienced Registered Nurse (RN) to join the team as a Hospice Case Manager , providing direct care and emotional support to patients and their families in the home setting.

Key Responsibilities:

Implement and oversee patient care plans in coordination with physicians and the interdisciplinary team Conduct ongoing assessments of patient and family needs, focusing on comfort, symptom management, and emotional support Educate and support family members on caregiving techniques and end-of-life care Supervise Hospice Aides and coordinate care with other providers and community resources Maintain accurate, timely documentation and attend regular team meetings Participate in on-call rotation, including evenings and weekends

Qualifications:

Active RN license in the state of Massachusetts Minimum 1 year of nursing experience (hospice or home health strongly preferred) Strong communication, organizational, and patient assessment skills Reliable vehicle and current auto insurance required Comfortable working independently in a home care setting
