The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. Duties Conduct thorough patient assessments to determine individual health needs and develop personalized care plans. Perform outpatient procedures including suture application and specimen collection processing. Collaborate with healthcare teams to facilitate effective discharge planning for patients transitioning from hospital to home care. Provide education to patients and families regarding health management, medication administration, and care protocols. Maintain accurate documentation of patient progress and coding in compliance with healthcare regulations. Utilize various systems for patient management and record keeping to ensure continuity of care. Offer specialized care for toddlers as needed, ensuring a safe and nurturing environment. Experience Current Registered Nurse (RN) license in the state of practice. Previous experience in home health or outpatient settings is preferred but not required. Proficiency in patient assessment techniques and familiarity with discharge planning processes. Experience with specimen collection processing and coding systems is a plus. Strong interpersonal skills to effectively communicate with patients, families, and healthcare professionals.
