RN, Acute Rehab Nora Concierge Care Charlestown, MA 02129 Job Overview We are seeking a dedicated and compassionate Registered Nurse specializing in Rehabilitation to join our dynamic healthcare team. The ideal candidate will possess a strong background in acute care settings, with experience in case management and home care. This role is pivotal in providing high-quality nursing care to patients recovering from various conditions, ensuring their rehabilitation process is effective and supportive. Duties Assess and monitor patients’ health status, including vital signs and overall well-being. Develop and implement individualized care plans based on patient assessments and rehabilitation goals. Provide skilled nursing interventions, including catheter care and medical documentation. Collaborate with interdisciplinary teams to coordinate patient care effectively within the hospital or teaching hospital environment. Educate patients and their families about rehabilitation processes, home care strategies, and self-management techniques. Utilize neonatal ventilators when necessary for patients requiring respiratory support. Conduct laboratory tests as needed to monitor patient progress and adjust treatment plans accordingly. Maintain accurate medical records and documentation to ensure compliance with healthcare regulations and standards. Requirements Current Registered Nurse (RN) licensure in the state of practice and BLS certification. Experience in acute care settings, preferably within rehabilitation or similar specialties. Proficiency in medical documentation practices and familiarity with electronic health records (EHR). Strong skills in case management, with the ability to advocate for patient needs effectively. Knowledge of home care practices and rehabilitation techniques. Experience or training in mammography is a plus but not required. Excellent communication skills, both verbal and written, to facilitate patient education and collaboration with healthcare teams. Ability to work effectively under pressure while maintaining a compassionate approach towards patient care.
