(BOSTON AREA) Job Summary Alliance Home Care VNA is searching for a Skilled Nurse to support and manage our patients in the Boston area. Candidate (s) must be an individual (s) who is warm, focused, professional, compassionate, communicative, talented, experienced, and reliable. (We offer benefits for full-time and/or a higher visit pay scale to our nurses) Full Job Description Under the direction of the DON the RN Case Manager will manage and oversee a caseload of patients in the Boston area which require delivering comfort and excellent care as required. Responsibilities and Duties Provide skilled nursing care to adult patients in a home setting Implement, develop, and document the plan of care to ensure quality and continuity of care for each patient with admissions, recerts and resumptions of care and supervisory visits. Provide effective communication to patient/family, team members, and other healthcare professionals involved Implement plan of care and treatment using sound judgment and critical thinking Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process Administer oral, IM, subacute prescribed medications, also treatments, and therapies Participate in staff meetings to exchange ideas and information as it relates to the quality initiatives. Qualifications and Skills Current Registered Nurse in the state of MA Minimum 3+ years of clinical home care experience Exceptional listening and analytical skills required. Excellent attention to detail. Work positively and productively as part of a team. Work flexible hours when needed. Must demonstrate excellent notes and documentation skills. Knowledge of HIPAA regulations Work independently. Current CPR accredited by American Heart Association and/or American Red Cross (certificate must be healthcare provider or healthcare professional). Valid MA Drivers license
