Position Summary:

We are seeking a Travel Emergency Department Case Manager (LPN or RN) for a contract assignment in Hyannis, MA. This role is essential in managing patient care coordination, discharge planning, and utilization review in a busy emergency setting. Ideal candidates are proactive, detail-oriented, and experienced in both clinical care and insurance protocols.

Key Responsibilities:

Perform daily screenings for discharge planning needs Complete and update InterQual® assessments Validate inpatient vs. observation status Coordinate with insurers for authorizations and documentation Issue payer notices and support appeal processes Collaborate with care teams to develop safe discharge plans Document all activities accurately in the EHR Participate in ED rounds and facilitate timely transfers Advocate for patients and ensure continuity of care Stay updated on case management protocols and participate in team initiatives

Qualifications:

Graduate of an accredited nursing program (LPN or RN) Active Massachusetts nursing license (LPN or RN) Bachelor’s degree in Nursing (BSN) preferred Case Management Certification (CCM, CPUR, or CPUM) preferred At least 3 years of recent acute care experience Minimum 2 years in case management, discharge planning, or utilization review Proficient with InterQual® or similar tools Strong communication, organizational, and EHR skills
