Principle Duties/Essential Functions:
As a Case Manager, you will play a vital role in ensuring seamless transitions and optimal outcomes for our residents.
Your responsibilities will include:
Care Coordination:
Collaborating with physicians, the interdisciplinary team, and external agencies to orchestrate comprehensive care plans and secure necessary resources for patients and their families.
Communication Hub:
Maintaining regular and effective communication with unit managers, therapy services, MDS coordinators, and social services to ensure a holistic approach to patient care.
Reporting & Analysis:
Generating and meticulously maintaining ACO, ALOS, and hospitalization reports to track key performance indicators.
Meeting Facilitation:
Leading and actively participating in Discharge Planning Meetings and Utilization Meetings to streamline processes and optimize resource allocation.
Regulatory Compliance:
Identifying and issuing Medicare non-coverage letters for Part A and managed care beneficiaries. Issuing Residents’ Rights notifications regarding transfers and notices of transfer/discharge to residents, ensuring copies are available for ombudsman scanning.
Medical Resource Management:
Overseeing all medical equipment and referral needs, including ordering DME (O2, nebulizer, wound vac) and making VNA/Home Care referrals as necessary.
Qualifications:
Education:
Bachelor’s Degree preferred.
Licensure:
Currently licensed as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Massachusetts – Required .
Experience:
At least five (5) years of Case Management experience in a healthcare setting is preferred.
Certifications:
