What You’ll Do:
Personalized Care:
Conduct in-home patient assessments to develop and implement individualized care plans.
Top-Quality Nursing:
Provide direct nursing care, including wound care, medication administration, and chronic disease management.
Educate & Empower:
Teach patients and their families about health management and self-care practices.
Collaborate:
Work closely with physicians and other healthcare professionals to ensure coordinated and comprehensive care.
Document & Report:
Maintain accurate and timely clinical records in compliance with regulations.
What You Bring:
Licensed RN:
Current and unrestricted RN license in MA.
Preferred Experience:
At least 1-2 years of nursing experience, preferably in home health. Willing to train the right person!
Flexibility:
Ability to work independently and adapt to varying patient needs and schedules.
Strong Communication:
Excellent interpersonal skills to effectively interact with patients and their families.
Tech-Savvy:
Proficiency with electronic medical records (EMR) systems.
