A respected healthcare organization in South Dennis, MA is seeking a compassionate and experienced Travel Registered Nurse (RN) Case Manager to join its Home Health team. This role offers a rewarding opportunity to deliver high-quality, patient-centered care in a home-based setting.
Key Responsibilities:
Deliver skilled nursing care in patients’ homes in accordance with physician orders and established care plans Develop, implement, and revise individualized care plans in collaboration with interdisciplinary teams Conduct comprehensive assessments and monitor changes in patient conditions Educate patients and families on disease management, medications, and home care techniques Ensure timely and accurate documentation in compliance with state, federal, and organizational guidelines Coordinate care with physicians, caregivers, and healthcare professionals to support continuity and quality of care
Qualifications:
Active Registered Nurse (RN) license in Massachusetts Minimum 1 year of clinical nursing experience; prior home health or hospice experience preferred Proficient clinical assessment and critical thinking abilities Strong communication, interpersonal, and organizational skills Reliable transportation and a valid driver’s license required
