Job Summary:
We are seeking a dedicated and compassionate Recovery Coordinator (Alcohol and Addiction RN) to provide support and guidance to individuals struggling with alcohol and addiction issues. The ideal candidate will work closely with clients to create personalized recovery plans and connect them with the necessary resources for overcoming their challenges.
Responsibilities and Duties:
Conduct comprehensive assessments of clients to determine their specific needs and goals for recovery Develop and implement individualized treatment plans in collaboration with clients and their support systems Provide education and counseling on addiction, relapse prevention, and coping strategies Coordinate and monitor client progress, adjusting treatment plans as needed Collaborate with other healthcare professionals, including therapists, doctors, and social workers, to ensure comprehensive care Maintain accurate and confidential client records and documentation Stay up-to-date on the latest research and best practices in addiction treatment
Qualifications and Skills:
