POSITION SUMMARY

Utilizing the nursing process, provides highquality direct and indirect patientcentered care. Coordinatespatient care by working collaboratively with the interdisciplinaryteam, patients and families. Communicates, as appropriate, thepatients clinical condition, responding quickly and accurately tochanges in condition or response to treatment. Demonstratesknowledge of the principles of growth and development over the lifespan and the skills necessary to provide age appropriate care tothe patient population served. Promotes the profession of nursingand professional practice through collegial support andinteractions.

EDUCATION/CERTIFICATION

Graduate of an accreditedschool of Nursing. Current licensure in the state of Connecticut.

Must provide copies of all current clinical competencies upon hireor transfer and during employment. Maintains clinical competencyrequired for area of assignment including but not limited to

BLS.EXPERIENCE

Minimum one year of nursing experience required.

COMPETENCIES

Able to communicate effectively in English bothverbally and in writing. Possesses basic computer skills. Must beable to work at both Manchester Memorial Hospital and RockvilleGeneral Hospital.

Job-Specific Competency Performs population-specific comprehensiveassessments and re-assessments of the patients condition and careneeds including the psychosocial, physical, educational andcultural aspects. Demonstrates knowledge of the principles ofgrowth and development over the life span, including adolescent,adult and geriatric population, and the skills necessary to provideage-appropriate care to patients served. Able to interpret dataabout the patient status in order to identify each patientsage-specific needs and provide care needed by the patient group.

Assesses and reassesses pain. Utilizes appropriate pain managementtools and techniques. Educates the patient and family regardingpain management. Documentation reflects assessments, interventionsand evaluation of effectiveness. Develops and revisesindividualized plans of care as indicated by assessment and patientresponse to treatment. Evaluates overall plan daily foreffectiveness. Involves the patient and family in the formulationof goals. Follows the seven (7) medication rights and reduces thepotential for medication errors. Demonstrates knowledge ofmedications and their correct administration based on age of thepatient and their clinical condition. Performs all aspects ofpatient care in an environment that optimizes patient safety andpromotes positive patient outcomes. Adheres to and practices safetybehaviors (CHAMP). Documents the nursing process and responses tocare in a timely manner utilizing the appropriate tools.

Demonstrates an ability to be flexible, organized and functionunder stressful situations. Utilizes critical thinking skills andsound judgment in priority setting and delegation. Formulates andimplements a teaching plan for patients and families based uponidentified learning needs. Evaluates effectiveness of learning.

Demonstrates autonomous practice, consistent with evidence-basedstandards. Pursues personal and professional growth anddevelopment. Serves as a professional role model and mentor.

Provides an accurate and pertinent report of the patients conditionfor safe transfer of care. Promotes and participates in qualityimprovement initiatives related to patient experience quality andsafety.
