General Summary of Duties:

The provision ofintermittent nursing service to patients in their homes under aplan of care established in collaboration with the patientsphysician and according to agency policies and procedures.

Supervision Received:

Reports directly to a Clinical Manager.

Supervision Exercised:

Home Care Aides.

Education:

Graduate of anNLN-approved school of nursing, associate or baccalaureate degreeprogram.

Experience:

One (1) year of recent medical/surgicalnursing experience, additional experience in community healthnursing preferred.

Certificate/License:

Possession of a currentlicense to practice as a Registered Nurse in the State ofConnecticut.

Typical Physical Demands:

Requires full range of bodymotion including handling and lifting patients, manual and fingerdexterity, and eye-hand coordination. Requires standing, walking,and the ability to ascend and descend stairs. Requires operation ofa motor vehicle. Occasionally lifts and carries items weighing upto 30 pounds. Requires corrected vision and hearing to normalrange. Requires working under stressful conditions. Requires someexposure to communicable diseases and/or bodily fluids.

TypicalWork Conditions:

Work is performed in patients homes and in anoffice environment. Some exposure to communicable diseases, toxicsubstances, and medicinal preparations.

Knowledge, Skills, andRequirements:

Knowledge of professional nursing theory and practiceand the skills to practice and evaluate patient care. Knowledge ofAgency policies, regulations and procedures to administer patientcare. Knowledge of medical equipment currently used to providepatient care. Knowledge of common safety hazards and precautions toestablish a safe work environment. Skill in applying and modifyingthe principles, methods and techniques of professional nursing toprovide ongoing patient care. Skill in identifying patient-relatedproblems and recommending solutions. Skill in documentation inclinical records. Skill in establishing and maintaining effectiveworking relationships with patients, families, physicians,co-workers, and the community. Ability to maintain quality controlstandards. Ability to react calmly and effectively in emergencysituations. Ability to interpret, adapt and apply guidelines andprocedures. Ability to communicate clearly. Possess adequatetransportation to carry out job requirements. Must show proof ofsatisfactory automobile insurance with State of CT minimumcoverage, recommended amount of $100,000/300,000.Willingness toprovide service in any part of Agency geographic area, according toAgency need.

Essential Duties:

(This list may not include all theduties assigned.) 1.Admits/readmits patients and providesintermittent nursing service to patients and their families incompliance with agency policies and State and Federal Regulations,incorporating the following: A complete physical and psychosocialassessment. Develops and implements an appropriate care plan andtherapeutic goals with the patient/family. Evaluation of patientsafety on a 24-hour basis. Exercises appropriate clinical judgmentin coordination of patient care by all services involved. Ongoingevaluation of significant changes in the condition of the patientand of progress toward the therapeutic goals. Use of Agency andcommunity consultants/resources. Instruction of patient/family inprocedures or responsibilities they are assuming and optimumwellness practices. Planning for the patients discharge fromservice. 2.Establishes and maintains case mix weight, utilization,patient satisfaction that aligns with industry standards and agencyoperational objectives for quality care. OASIS documentationcorrelates with the Plan of Care. Uses recommended Oasis assessmenttechniques 100% of the time. OASIS, documentation, and plan of carecorrelate to 95%. 3. Documents all services provided in theclinical record in accordance with State and Federal Regulationsand Agency policy: Documentation, including activities and mileageis exported within 36 hours of visit or prior to next scheduledvisit. Obtains physician verbal/written orders and complies withall reporting requirements. Participates in coordination of carewith physicians and with other disciplines as appropriate. 4.Manages work assignment in effective and efficient manner: Visitsall new therapeutic clients within 24 hours of referral unlessotherwise specified by the physician. Reports any cases to theSupervisor of Clinical Services where goals can not be met orat-risk situations exist and responds appropriately. Remainsflexible in daily planning, allowing time for emergencies orunexpected visits. Maintains productivity as established by theAgency. Utilizes payer sources correctly, reflecting changinglevels of care. Delegates patient care responsibilities to HomeCare Nurse (LPN) as appropriate. Demonstrates an understanding ofthe principles of case management. Demonstrates ability to managecost and quality 5. Establishes and evaluates the Plan of Care tobe delegated to the Home Health Aide, orients and supervises theHome Health Aide in accordance with State and Federal Regulationsand Agency policy: Develops written plan of care for the HomeHealth Aide and updates this plan as patients condition warrants;plan of care must be reviewed every 60 days. Obtains input fromHome Health Aide regarding patient progress. Prepares writtenperformance evaluations of home care aides under his/hersupervision. Provides supervision of home care aides at a minimumof every 14 days or as required by payer source. 6. Develops andmaintains sufficient professional skills and credentials to meetthe needs of patient care: Attends a minimum of 12 hours ofin-service per year to include mandated annual in-services.

Participates actively in self-appraisal and the formation of a planof self-development. Adheres to Agency Corporate Compliance Planand HIPAA Privacy Plan as per written agreement. Seeks appropriateclinical input to ensure appropriate patient care. Projectsprofessional image through attitude and compliance with dress code.

Capable of precepting new nurses and students. Functions as a teammember with consideration to all staff. Provides Agency withcurrent copy of professional license. Maintains CPR certification.

Ancillary Duties 1. Performs other duties as necessary to supportAgency programs as assigned by the Supervisor of Clinical Services.

Agency committees as requested. Flu/special clinics as needed.

Elder Wellness Clinics as assigned. Special Agency or communityprograms during and/or after regular working hours. 2. Performsother duties as necessary to maintain current Agency personnelpolicies: Gives adequate time and attention to administrativeduties. Abides by Agency weekend/holiday rotation of nurses. Mayprovide back-up for on-call service. MPECHN
