MDS Coordinator ~ Full Time ~ Days Maplecrest Rehabilitation & Living Center 174 Main Street, Madison, ME 04950 MDS Coordinator Full Time ~ Days $10,000 Sign On Award Maplecrest Rehabilitation & Living Center, a healthcare facility located in Madison, has an immediate opening for an MDS Coordinator. Duties will include managing the entire MDS process, completing assessments and developing care plans.
The ideal candidate will have:
A friendly, team-oriented disposition Flexibility, and be detailed oriented Understanding of PDPM Experience in a long term care or rehabilitation setting RN required Experience with the MDS is preferred but not required State of Maine License
