Home Health RN Aptiva Healthcare York, ME Job Overview We are seeking a dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide compassionate and high-quality care to patients in the comfort of their homes. Duties Conduct patient assessments and develop individualized care plans Administer medications and treatments as prescribed Perform procedures such as IV insertion, tube feeding, and neonatal ventilator care Implement infection control measures to ensure patient safety Provide specialized care such as memory care, DME management, and botulinum toxin treatment Utilize electronic health records systems like Cerner for documentation Collaborate with healthcare professionals for diagnostic evaluations Experience Active Registered Nurse license Prior experience in home health or a related field is preferred Proficiency in various nursing skills including OR procedures and specialized treatments Strong communication and interpersonal skills Ability to work independently and as part of a multidisciplinary team
