•Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS
•ME/compact RN license; BLS; 2 years acute hospital case management experience
PREFERRED QUALIFICATIONS
•EPIC, utilization review, discharge planning, and care coordination of patients from the ED, ICU, and Med-Surg units experience
