Home Visit Nurse Practitioner | Kansas City, MO $130k-$150k
Direct Hire Job Summary:
We’re looking for a compassionate, experienced Advanced Practice Provider (APP) to deliver high-quality primary care to our senior members, both in our centers and in their homes. This role focuses on building lasting relationships, preventing illness, and improving quality of life for adults aged 60 and older.
Key Responsibilities:
In-Center Care:
Provide outpatient primary care services in collaboration with supervising physicians. Maintain accurate and timely patient records and documentation. Collaborate with the care team to deliver exceptional patient care. Help ensure short wait times and timely attention for urgent patient needs. Participate in clinical meetings, quality reviews, and community outreach.
In-Home Care:
Deliver medical services directly in members’ homes as needed. Identify and reach out to members in need of home visits. Coordinate with care team members to ensure continuity of care. Travel within your assigned area with reliable transportation and the ability to carry necessary medical equipment (up to 20 lbs).
Qualifications:
Active, unrestricted nurse practitioner or physician assistant license in the state of employment. Experience in internal medicine, geriatrics, or family medicine.
BLS/ACLS
certified. Passionate about serving Medicare-aged, medically underserved populations. Strong communication, diagnostic, and teamwork skills. Willing to travel between nearby centers and member homes.
