In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans that promote optimal health outcomes. Your expertise will be essential in managing complex medical conditions and facilitating a smooth transition from hospital to home. Responsibilities Conduct comprehensive patient assessments to determine healthcare needs and develop personalized care plans. Administer medications and treatments as prescribed, while monitoring patient responses and adjusting care as necessary. Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care effectively. Educate patients and their families about health management, disease prevention, and self-care techniques. Assist with discharge planning to ensure a seamless transition from hospital to home care. Maintain accurate and timely documentation of patient progress and interventions in compliance with regulatory standards. Utilize medical terminology effectively in communication with healthcare team members. Engage in utilization management practices to optimize patient outcomes within managed care frameworks. Experience Current Registered Nurse (RN) licensure in the state of practice. Experience in home health care or a related field is preferred but not mandatory. Familiarity with Level I and Level II Trauma Centers is beneficial for understanding complex patient needs. Knowledge of discharge planning processes and managed care principles is an asset. Strong skills in patient service, communication, and collaboration within interdisciplinary teams are essential for success in this role. Previous experience in emergency room (ER) settings or primary care environments can enhance your ability to manage acute situations effectively.
