Mary’s Medical Center- Blue Springs, MO The Nurse Case Manager in an acute care setting is responsible for coordinating patient care from admission through discharge. This role involves assessing patient needs, creating individualized care plans, and collaborating with healthcare teams to ensure efficient and effective care. The Nurse Case Manager strives to improve patient outcomes while optimizing resource utilization. The Case Manager will conduct comprehensive assessments of patients’ medical, psychosocial, and functional needs, develop individualized care plans in collaboration with multidisciplinary teams, and Coordinate and facilitate communication among patients, families, and healthcare providers. Responsibilities Essential Duties and Responsibilities (includes, but not limited to): Conduct thorough assessments of patients’ medical, psychological, and social needs.
Develop individualized care plans addressing health goals, resource utilization, and patient safety.
Schedule and coordinate medical appointments, therapies, and community services.
Advocate for patients’ needs and preferences.
Provide education to patients and families regarding medical conditions, treatment options, and self-care strategies.
Monitor and evaluate healthcare services to ensure they meet established criteria for medical necessity and efficiency.
Assist in managing medical resources and minimizing unnecessary costs.
Maintain accurate and comprehensive documentation of patient assessments, care plans, and outcomes.
Prepare reports and communicate effectively with the healthcare team for quality improvement. Qualifications Before we go any further, we do have some deal-breakers.
You must have:
Starting April 1 2015. Minimum 5 years work experience post-graduation of an accredited school of nursing and a current state Registered Nurse license.
Grandfathered prior to April 1, 2015. Minimum 5 years post graduate of an accredited school Of Social Work for Licensed Clinical Social Worker. However, RN Case Manager preferred.
Five years acute care nursing experience preferred. At least one year experience in case management, discharge planning or nursing management, preferred.
Current BCLS (AHA) certificate, preferred.
Knowledge of Milliman Criteria and InterQual Criteria preferred.
Experience and knowledge in basic to intermediate computer skills.
