Job Summary and Qualifications The CaseManager (CM) ensures high-quality, patient-centered care bymanaging Rehabilitative care to ensure optimum outcomes. The CMprovides, coordinates, and directs care specific to the needs ofeach Rehab patient. The CM collaborates with the Rehab ProgramDirector and Facility Case Management Director regardingdepartmental functions. The CM coordinates efforts within theRehabilitation team. What you will do in this role: You willprovide program orientation to patients/families/caregivers.

Orientation will include the case manager’s role, Rehab philosophy,and continued stay and discharge criteria. Orientation will alsoinclude Medicare and insurance benefits, grievance procedures,treatment plan process, and rights and responsibilities. You willact as the coordinator of patient/family/caregiver education. Youwill promote the participation of the patient/family/caregiver inteam discussions related to plans, goals, and status. This will beconducted through Family Conferences and other interactions. Youwill ensure the implementation of the patient’s treatment plan thatsupports the patient’s strengths, abilities, needs, andpreferences. You will facilitate the involvement of the patientthroughout the rehabilitation process. You will document thefindings of the Discharge Planning Evaluation (DPE) andpsychosocial assessments. You will communicate the social,financial, or discharge needs and preferences of thepatient/family/caregiver. You will assume accountability forpromoting consistent, positive patient interactions that advancethe agenda of unparalleled patient service.
