Full Job Description Position Summary:

The Hospice RN Case Manager is responsible for managing a patient caseload and providing high-quality, compassionate, end-of-life care in accordance with the patient’s plan of care. This role includes clinical assessment, care coordination, symptom management, patient and family education, and communication with the interdisciplinary team. The RN Case Manager ensures patients’ physical, emotional, and spiritual needs are met during the hospice journey.

Key Responsibilities:

Serve as the primary nurse for an assigned group of hospice patients. Conduct initial and ongoing comprehensive assessments of patient/family needs. Develop, implement, and regularly update individualized patient care plans. Provide skilled nursing care including pain and symptom management, medication administration, wound care, and crisis intervention. Coordinate care and communicate with physicians, aides, social workers, chaplains, and other team members. Educate patients and families on disease progression, medications, and care techniques. Ensure documentation is accurate, complete, and meets regulatory standards (EMR or paper charting). Participate in interdisciplinary team meetings and collaborate on patient care goals. Respond to emergency situations during regular hours and participate in the on-call rotation as required. Advocate for patients and support their dignity, preferences, and quality of life.

Qualifications:

Education:

Graduate of an accredited nursing program (ASN or BSN).

Licensure:

Current Registered Nurse (RN) license in the state of practice.

Experience:

Minimum 1 year of clinical nursing experience, hospice or palliative care preferred.

Certifications:
