Provides services as a patient navigator throughoutthe continuum of care.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Functions as a liaison for managementstaff, administration, physicians, managed care companies,community organizations, and other customers.
- Collaborates withphysicians and advanced practice providers to ensure qualityoutcomes. Coordinates efforts for treatment and promotes qualityimprovement and educational efforts.
- Compiles data, tracksoutcomes, and makes recommendations for process improvement.
- Provides referrals/support for follow-up care for patients who areuninsured or underinsured.
- Utilizes the nursing process ofassessment, planning, intervention, and evaluation in all patientencounters.
- Assists medical provider with processing ofdiagnostic results bringing significant values to the provider’sattention. Exhibits knowledge of tests and procedures and theirsignificance as related to patient care.
- Recognizes theimplementation of dosages, interactions, side effects, adverseeffects, routes of administration of drugs as they apply toindividual patients. Uses knowledge of medications in instructingpatients about medication therapy.
- Establishes workingrelationships teaching protocols for patient education, familycounseling and general public information.
- Works in a constantstate of alertness and safe manner.
- Performs other duties asassigned.
EDUCATION
- Graduate of accredited school of nursing oreducation equivalency for licensing
EXPERIENCE
- Three years’registered nurse experience
PHYSICAL REQUIREMENTS
- Constant use ofspeech to share information through oral communication.
- Constantstanding and walking.
- Frequent lifting/carrying andpushing/pulling objects weighing 0-25 lbs.
- Frequent sitting,reaching and keyboard use/data entry.
- Frequent use of vision anddepth perception for distances near (20 inches or less) and far (20feet or more) and to identify and distinguish colors.
- Frequentuse of smell to detect/recognize odors.
- Frequent use of hearingto receive oral communication, distinguish body sounds and/or hearalarms, malfunctioning machinery, etc.
- Occasionallifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Occasional lifting/moving of patients.
- Occasional bending,stooping, kneeling, squatting, twisting, gripping and repetitivefoot/leg and hand/arm movements.
- Occasional driving.
- Rarecrawling and running.
