He/she functions as a member of the coordinated care team. The team consists of Social Workers, Case Managers, Nurse Managers, Ancillary departments, and Staff Nurses. He/she is responsible and accountable for day to day concurrent clinical resource utilization of assigned patient population and the collection of clinical, quality and financial outcome information for that population.
Job Function :
Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards. Collaborates with physicians and other team members to progress patient to the most appropriate level of care. Intervene with potential over utilization of clinical resources. Facilitate timely discharge planning. Facilitate education, social, nutritional, etc., assessments and interventions. Attends Patient Care Conferences. Assist in the development, implementation and evaluation of clinical paths. Assist with analyzing variances and disseminating outcome data to the patient care team. Communicates clinical information to 3rd party payors in a timely manner in accordance with their requirements. Prolonged periods of standing, walking, or sitting while on duty. Lifting, pushing, and pulling up to 50 pounds with or without assistance. Transferring, repositioning patients. Reaching, stooping, bending, kneeing, and crouching for patient care functions. Visual and auditory acuity and manual dexterity essential to performing designated duties required. Optimal auditory acuity required. Manual dexterity involving the handling of equipment and instruments or needles is essential to performing assigned duties. Ability to perform effectively in a stressful and fast-paced environment. Ability to pass all required health and other screening tests. Physical conditions are clean, neat and well lit. Maybe subjected to unpleasant sights and odors, stressful situations and hazardous or infectious agents where judgment as to precautions need to be taken is essential. Climate control and ambient temperature variances may be experienced. Exposure to limited amounts of hazardous chemicals or substances and infectious disease processes are a possibility. Hours of duty maybe irregular or unexpectedly extended due to emergency circumstances. Direct contact with blood or other body fluids to which universal precautions apply. Personal protective equipment is made available and must be worn.
Education :
Degree from an accredited school of nursing.
Work Experience :
Minimum of two years’ clinical expertise required. Case management or Utilization management experience preferred. Employee must demonstrate ability to recognize patients’ individual needs based on medical conditions, age (infants, pediatrics, adolescents, young adults, middle-aged and geriatric), limitations and planned procedures. Requires oral and written communication skills.
