Job Requirements
2 years of acute case management experience
6 months of experience in a Level 1 trauma or large academic health system, with exposure to multiple inpatient floors
Proficiency in Epic EMR system
Responsibilities
Manage a caseload of 15-20 patients, coordinating discharge planning and care transitions
Collaborate with a multidisciplinary team, including social workers, to develop and implement patient care plans
Utilize Epic Care Management and Navihealth for referral processes
