Job Description Summary Manages individualized, outcome orientednursing care through the use of the nursing process. Provides care,treatment and services through the successful coordination andcompletion of a series of processes that include assessment,planning care, providing care, coordinating cross continuum careand assessing outcomes of interventions in accordance with system,departmental and ambulatory policies and procedures. This role provides clinically-based, risk reducing care andcoordination that facilitates the delivery of cost-effectivequality healthcare along the continuum. It directly interfaces withphysicians, care managers, health care teams, patients and theircaregivers to achieve desired patient outcomes. Excellentcommunication skills, both written and verbal, a commitment tocoaching patients on health promoting behaviors, and an ability tolisten and assimilate complex clinical information are required inthe role. The ability to work autonomously and in teams isessential. ESSENTIAL FUNCTIONS

- Demonstrates knowledge of and follows SSHpolicies and procedures for administering and documentingmedications. b
- Completes medication reconciliation processfollowing SSH policy and procedure.
- c•Demonstrates safeprocedures for the safe administration of medication, treatmentsand injection/vaccines 2•Plan of care/Documentation/PatientFamily Centered Care/Patient Experience Develops, discusses, andcommunicates a prioritized problem list/plan of care for eachpatient .

- Develops, evaluates and updates individualized planfor patient and documents outcomes. b
- Initiates patient outreach,identifies and documents patient/family/significant other needs andmakes appropriate interventions, evaluating outcomes ofinterventions. c
- Continues assessment/reassessment and identifiescare needs within established nursing practice. d
- Documents allpatient care following the department of nursing policy,department-based standards, disease processes (CHF, PNA vaccines)and nurse sensitive indicators (falls, safety risk, isolation,etc.) e
- Assess/reassesses and documents patient’s response toeach intervention both pharmacological and non-pharmacological perSouth Shore Health and ambulatory policies and procedures. f
- Works on behalf of patient /family. Seeks help to representpatient/family when they are unable to represent themselves. g
- Raises ethical questions and concerns with clinical team. Seeksavailable resources to help formulate and understand ethicaldecisions. h

- Safety/Quality
- Foster’s a “Culture of Safety” throughpersonal ownership and commitment to a safe environment. a
- Verifies patient identification with two identifiers prior to thestart of any procedure, including “time out”, administration ofcare, medications, labeled specimen’s and documents confirming thecorrect patient, procedure, site, equipment and consent. b
- Complies with the current CDC hand hygiene guidelines throughproper handwashing. Adheres to universal precautions, makesappropriate use of personal protective equipment at all times andappropriately disposes of hazardous materials. Maintains awarenessof MSDS sheets and how to access. c

- Demonstrates the ability to care to patients receiving urologiccare in the ambulatory setting according to SSH policy andprocedure. 2
- Complete all specialty based competencies forurology care and oncology care prior to deadlines.
3-Functions inthe role of charge or resource as assigned by the nursesupervisor/leader. 4-Consistently abides by SSH infection controlprocedures including hand hygiene and sterile technique. 5-Attendsclinic/department based staff meetings and employee forums.6-Participates in at least one specialty based initiative,committee, or hospital committee. And
