Harrison’s Hope Hospice is hiring a RN Navigator to support ourMeridian team. Come join our compassionate team of professionalclinicians who make a difference in the lives of patients andfamilies facing an end-of-life journey. This position isresponsible for collaboration with the Hospice Liaison team andAdmissions team to increase public awareness of hospice and otherservices provided by the agency and to drive admissions for theplacement of patients to meet expectations. This role will act asfirst point of contact for hospitals, health care facilities, andother sources regarding referrals, will seek networkingopportunities to provide education and to develop relationshipswith new and existing referral sources. In addition, this role willbe responsible for identifying appropriate care and services topatient and family in accordance with attending physician and planof care, as well as company policy and procedures. POSITION REQUIREMENTS
: Bachelor’s degree inmarketing, business administration, or other related fields ispreferred. An acceptable combination of experience and/or trainingmay be considered instead of formal education.
