Full-time Position Overview:

North Central PDD is seeking a dedicated and compassionate Nurse Case Manager to join our Elderly and Disabled (E&D) Waiver program team. This role plays a critical part in helping seniors and individuals with disabilities maintain their independence and receive care in their homes and communities.

Key Responsibilities:

Conduct comprehensive assessments and develop individualized care plans for waiver participants. Coordinate and monitor home- and community-based services to ensure quality and compliance. Work collaboratively with families, caregivers, providers, and other healthcare professionals. Ensure documentation and case records are accurate and up to date according to state and federal guidelines. Perform in-home visits as required to assess client needs and service delivery.

Minimum Qualifications:

Registered Nurse (RN) license in the state of [insert state]. Minimum of 2 years of nursing experience, preferably in geriatrics, community health, or case management. Knowledge of Medicaid Waiver programs and regulations is preferred. Strong interpersonal, organizational, and communication skills. Proficiency in electronic documentation and Microsoft Office Suite. Must have a valid driver’s license and reliable transportation for travel within the region.
