Coordinates support to providers by anticipating the needs and providing assistance in the provision of care. Coordinates services with other members of the health-care team, internal and external, to provide complex patient care, including the administration of intravenous infusions and wound care. Assists in maintaining an inventory of supplies and medications and orders as needed. Ensures all quality assurance measures are achieved. Completes all other duties as assigned.
Education/Experience :
Graduate of an accredited school of nursing.
Preferred:
Has practiced nursing in the last 3 years. Prefer experience in home care or long-term care setting, and ideally, PACE experience. Two years of acute care management experience is preferred and/or 1 year of community-based nursing experience. Must possess knowledge and skills necessary to treat patients Meet a standardized set of competencies for the specific position established by PHSC and approved by CMS before working independently Excellent interpersonal, oral and written communication skills
