Schedule:
Monday-Friday 8a-5p with two holidays on call per year

Assume primaryresponsibility for patient and family caseload, including theassessment, determining eligibility, admission processing,planning, implementation, and evaluation phases of the nursingprocess Obtain data and assess necessary information from patientphysical, psychological, social, and spiritual factors that mayimpact the patient and family’s needs and initiate intervention byother members of the interdisciplinary team Initiate communicationwith the attending physician, interdisciplinary team, and otheragencies for the purpose of coordinating optimal care Provideregular visits to re-assess the patient’s needs and update the planof care Perform clinical pronouncement of death and provide supportto the family Maintain accurate patient records, charts, progressnotes, reports and prepare care plans timely Supervise and documentdelivery of care by the Home Health Aide as required by Federal,State, and Company guidelines
Qualifications:
