Job Summary Accountable for the execution, adaptation and outcome of caremanagement as prescribed in the initial Utilization Review Planwith regard to resources, appropriateness of care and adherence toa designated Geometric Length of Stay (GLOS) for each patient.

Essential Functions 1. Identifies and implements placement of services to facilitatesuccessful patient outcomes and highlighting patients at high tomoderate risk for readmission. Follows all core measures diagnosisfor readmission. 2. Serves as the contact for patients, families, and communityliaisons to ensure coordination of post-acute services. Run dailyreadmission reports. 3. Provides support and education to the patient and families bylistening and responding to their questions/concerns about thedischarge process and educating them about the options that areavailable for post discharge care. Begin the coachingrelationship. 4. Assists with the referral process based on patient and familychoice as well as the ability of the post discharge facility tomeet the specific needs of the patient. 5. Promote effective utilization of healthcare resources forplacement in the appropriate level of care post discharge byfacilitating updates to patients discharge plan. 6. Work collaboratively with the interdisciplinary healthcare team,internal and external, to achieve optimal clinical outcomes byfollowing patient for 30 days post discharge. 7. Collaborate with physicians, patients, and families in theafter-care planning process to ensure all critical elements havebeen communicated and addressed. 8. Document all discharge planning activities in Cerner and reviewdata to ensure accuracy with the goal of improving the clinicalexperience for referred patients and their physician. Manageresources appropriately (scales, med planners, medicationvouchers). 9. Strives to improve the discharge process for patients and theirphysician by monitoring the hospital patient satisfaction andphysician satisfaction scores 10. Monitor regulatory issues and requirements regarding denial ofbenefits and provide information to appropriate individual. Contactpayer to assess benefit coverage and the need for priorauthorization for post discharge care. 11. Communicate with the physician and facilitate/expedite thedischarge when patient is ready for lower level of care. 12. Attendance Policy 13. Qualifications:

Certifications/ Licenses Associates Degree in Nursing required Bachelor’s Degree in Nursing preferred. Requires critical thinking skills, decisive judgment and theability to work with minimal supervision. Must be able to work in astressful environment and take appropriate action.

Licenses:

Current RN license to practice in NC (NC licensure ormulti-state (compact) license). NIH Stroke Scale required. Minimum Work Experience 1-3 years case management experience preferred. Two years hospital based clinical experience. Experience in CaseManagement, Utilization Review and/or discharge planning, knowledgeof Interqual and other payor criteria preferred. Must haveexcellent communication skills, demonstrate self-direction, andhave the ability to work well in a team environment
