Accountable for the execution, adaptation and outcome of caremanagement as prescribed in the initial Utilization Review Planwith regard to resources, appropriateness of care and adherence toa designated Geometric Length of Stay (GLOS) for each patient. Frye Regional Medical Center, located in the beautiful foothills ofNorth Carolina is a 355-bed acute care hospital, 81-bed BehavioralHealth facility and more than 70 primary and specialty careproviders. Identifies and implements placement of services to facilitatesuccessful patient outcomes and highlighting patients at high tomoderate risk for readmission. Serves as the contact for patients,families, and community liaisons to ensure coordination ofpost-acute services. Provides support and education to the patientand families by listening and responding to theirquestions/concerns about the discharge process and educating themabout the options that are available for post discharge care.

Assists with the referral process based on patient and familychoice as well as the ability of the post discharge facility tomeet the specific needs of the patient. Promote effective utilization of healthcare resources for placementin the appropriate level of care post discharge by facilitatingupdates to patients discharge plan. Work collaboratively with the interdisciplinary healthcare team,internal and external, to achieve optimal clinical outcomes byfollowing patient for 30 days post discharge. Collaborate with physicians, patients, and families in theafter-care planning process to ensure all critical elements havebeen communicated and addressed. Document all discharge planning activities in Cerner and reviewdata to ensure accuracy with the goal of improving the clinicalexperience for referred patients and their physician. Strives toimprove the discharge process for patients and their physician bymonitoring the hospital patient satisfaction and physiciansatisfaction scores Monitor regulatory issues and requirements regarding denial ofbenefits and provide information to appropriate individual. Contactpayer to assess benefit coverage and the need for priorauthorization for post discharge care. Communicate with the physician and facilitate/expedite thedischarge when patient is ready for lower level of care. Licenses:

Current RNlicense to practice in NC (NC licensure or multi-state (compact)license). Minimum Work Experience Two years hospital based clinical experience. Experience in CaseManagement, Utilization Review and/or discharge planning, knowledgeof Interqual and other payor criteria preferred.
