Home Health Role:

Conduct comprehensive assessments of patients’ health conditions and develop individualized care plans Administer medications, treatments, and other nursing interventions as prescribed by physicians or healthcare providers Monitor vital signs, assess and document patient progress, and adjust care plans accordingly Collaborate with interdisciplinary healthcare team members to provide coordinated and effective patient care Educate patients and their families about disease management, self-care techniques, and medication administration Communicate effectively and professionally with patients, caregivers, and healthcare personnel to ensure continuity of care Adhere to all relevant laws, regulations, and ethical standards in nursing practice Establish and maintain accurate medical records, including patient assessments, care plans, and progress notes Participate in performance improvement initiatives to enhance the quality of care and promote positive patient outcomes

Requirements:
