Working closely with a dedicated team of physicians and clinicians,you’ll develop and manage care plans that support recovery and helppatients get back to the life they love. As a Home Health RN CaseManager , you will: Provide admission, case management, andfollow-up skilled nursing visits for home health patients.
Administer on-going care and case management for each patient,provide necessary follow-up as directed by the Clinical Manager.
Confer with physician in developing the initial plan of treatmentbased on physician’s orders and initial patient assessment. Providehands-on care, management, and evaluation of the care plan andteaching of the patient in accordance with physician orders, underClinical Manager’s supervision. Revise plan in consultation withphysician based on ongoing assessments and as required bypolicy/regulation. Coordinate appropriate care, encompassingvarious healthcare personnel (such as Physical Therapists,Occupational Therapists, Home Health Aides, and externalproviders). Report patient care/condition/progress to patient’sphysician and Clinical Manager on a continuous basis. Implementpatient care plan in conjunction with patient and family to assistthem in achieving optimal resolution of needs/problems.
Coordinate/oversee/supervise the work of Home Health Aides,Certified Home Health Aides and Personal Care Workers and provideswritten personal care instructions/care plan that reflects currentplan of care. Monitor the appropriate completion of documentationby home health aides/personal care workers as part of thesupervisory/leadership responsibility. Discharge patients afterconsultation with the physician and Clinical Manager, preparing andcompleting needed clinical documentation. Prepare appropriatemedical documentation on all patients, including any caseconferences, patient contacts, medication order changes,re-certifications, progress updates, and care plan changes. Preparevisit/shift reports, updates/summarizes patient records and conferswith other health care disciplines in providing optimum patientcare Use your skills to make an impact
Required Experience/Skills:
Diploma, Associate, or Bachelor Degree in Nursing A minimum of oneyear of nursing experience preferred Strong med surg, ICU, ER,acute experience Home Health experience is a plus Current andunrestricted Registered Nurse licensure Current CPR certificationStrong organizational and communication skills A valid driver’slicense, auto insurance, and reliable transportation are required.

- $96,900 per year base pay Scheduled Weekly Hours 40 Pay Range Thecompensation range below reflects a good faith estimate of startingbase pay for full time (40 hours per week) employment at the timeof posting.
The pay range may be higher or lower based ongeographic location and individual pay will vary based ondemonstrated job related skills, knowledge, experience, education,certifications, etc. $70,500
